To ensure the best possible match for you and your new furry friend, 2nd Chance Cat Rescue requires a questionnaire to be completed prior to the adoption being finalised.

To complete the adoption questionnaire, please click here to begin.

All mandatory fields must be completed to activate the Submit button (an error message will display next to the missed field as a guide). Once you have clicked on the Submit button, a notification popup will show, indicating that your form has been submitted correctly, then the form will clear. If you have not heard from us or one of our foster carers within 24 hours, please message us via our Contact Form to ensure we received your completed questionnaire.

On Adoption day you will need to pay the fee stated on the cat’s profile.
